About the Role
We’re seeking a passionate and driven Hospital Pharmacist to join our team at Slade Pharmacy, located onsite at St Vincent’s Private Hospital in Fitzroy. This is a permanent full-time position, offering the opportunity to make a meaningful impact on patient care while working in a supportive and collaborative hospital environment.
In this role, you’ll be responsible for delivering high-quality clinical pharmacy services across both inpatient and discharge settings. You’ll work closely with a multidisciplinary team to ensure safe, effective, and personalised medication management throughout the patient journey. From conducting medication chart reviews and reconciliations to counselling patients on discharge medications, your expertise will directly contribute to improved health outcomes.
You’ll also play a key role in supporting the day-to-day operations of the pharmacy, including dispensing, inventory coordination, and liaising with hospital staff to ensure seamless service delivery. Whether you're an experienced hospital pharmacist or transitioning from community pharmacy, this role offers a fantastic opportunity to expand your clinical skills in a dynamic and rewarding setting.
Roster:
This is a permanent full-time position, working from Tuesday to Saturday (76 hours per fortnight). Start and finish times are as follows:
- Tuesday: 8:00am to 4:30pm
- Wednesday: 8:00am to 4:00pm
- Thursday: 8:00am to 4:00pm
- Friday: 8:00am to 5:00pm
- Saturday: 8:00am to 3:00pm
Key Responsibilities:
- Dispense and supply medication and pharmacy products
- Coordinate patient medication requirements for inpatient stay and discharge
- Conduct medication chart reviews and medication reconciliation
- Develop and maintain professional relationships with key stakeholders to ensure excellent customer service to clients and patients
- Support patients, carers, hospital staff and associated employees
- Educate and advise staff and customers on health promotion, disease prevention, drug selection and the proper use of medicines
About You
We’re looking for a passionate and adaptable pharmacist who is committed to delivering modern, patient-focused care and thrives in a collaborative hospital environment.
Key Requirements:
- Current AHPRA General Registration as a Pharmacist
- Experience in an Australian hospital or community pharmacy
- Familiarity with Fred Dispense and PBS (preferred)
- Strong communication and interpersonal skills
- Excellent time management and attention to detail
- Ability to work independently and as part of a team
- A proactive mindset with a commitment to continuous learning and improvement
