Telehealth Hospital Pharmacist - Slade Pharmacy Whyalla, South Australia




About the Role

We are seeking a motivated and clinically focused Telehealth Hospital Pharmacist to join Slade Pharmacy Whyalla. This position is offered on a part-time or full-time basis and will be engaged on a fixed-term contract, with duration shaped by operational requirements and discussed in alignment with the successful candidate.
 
This is a unique opportunity to deliver comprehensive hospital pharmacy services predominantly from home while remaining closely connected to a regional hospital pharmacy team. In this role, you will perform almost all core hospital pharmacist duties remotely, supporting inpatient and discharge medication management, dispensing, and patient counselling via phone and telehealth platforms. You will liaise closely with doctors, nurses, and allied health professionals, providing clinical advice and ensuring safe, effective, and timely medication use across the patient journey.

While the role is primarily remote, some travel to Slade Pharmacy Whyalla will be required to support onboarding and training, leave coverage, team engagement, or specific operational needs. This role offers an innovative and flexible way to practise hospital pharmacy, combining meaningful patient impact with the benefits of remote work, while contributing to the delivery of high-quality pharmacy services in a regional hospital setting.

Key Responsibilities:
  • Conduct medication chart reviews and medication reconciliation using hospital clinical systems
  • Coordinate patient medication requirements throughout admission and at discharge
  • Provide patient counselling and medication education via phone and telehealth
  • Liaise with prescribers, nurses, and hospital staff to support safe and effective medication management
  • Provide clinical advice on medication selection, dosing, interactions, and adverse effects
  • Document clinical interventions accurately and in line with hospital and governance requirements
  • Participate in occasional onsite duties at Slade Pharmacy Whyalla as required for training, leave coverage, or operational needs
 
About You
You are a confident and adaptable hospital pharmacist who enjoys working independently while remaining connected to a broader clinical team. You are comfortable using digital systems, value clear communication, and are committed to delivering high quality, patient focused care through telehealth models. You are excited by the opportunity to contribute to regional healthcare in a flexible and innovative way.
  
Key Requirements:
  • Current AHPRA General Registration as a Pharmacist
  • Experience working in an Australian hospital or community pharmacy setting
  • Demonstrated clinical pharmacy knowledge and sound professional judgement
  • Strong verbal and written communication skills, particularly via phone or virtual platforms
  • Ability to work autonomously while collaborating effectively with a remote team
  • Excellent time management, accuracy, and attention to detail
  • Competency using pharmacy dispensing and hospital clinical systems, Fred Dispense preferred
  • Willingness and ability to travel to Whyalla occasionally as required
