We are seeking a Palliative Care Pharmacist to join our team at Mater Hospital North Sydney, supporting a newly established palliative care unit.
This is the first dedicated role of its kind at this site, offering a unique opportunity to contribute to the development of pharmacy services within palliative care. You will work closely with a multidisciplinary team to support patients with complex therapeutic needs and play a key role in improving quality of care and patient outcomes.

About the Role
Palliative care services at Mater North Sydney have previously been supported by the broader pharmacy team. This role introduces dedicated clinical pharmacy support, strengthening medication management within this specialised area.
Key responsibilities:
- Conducting medication reconciliation and medication reviews
- Supporting symptom management strategies through medication optimisation
- Providing advice on pain management, end-of-life care, and deprescribing where appropriate
- Working collaboratively with medical, nursing, and allied health teams
- Supporting safe and effective medication use across the patient journey
- Educating patients, carers, and staff on medication use and best practice
- Assist access to specialist palliative care medicines via SAS
- Liaise with community team and hospitals to provide smooth transition of care.
Shift information:
- 19-hours a week
- Preferred days: Monday, Tuesday and Wednesday
- Flexible work pattern with 3x 6-hour shifts, or 4x shorter shifts
- Negotiable start/finish times
About You
You are a clinically capable Pharmacist with a passion for patient-centred care, particularly in complex or supportive care settings.
Hospital experience is beneficial but not required, we welcome applications from pharmacists across community or other clinical settings who are looking to transition into hospital or palliative care-focused practice.
Eligibility requirements:
- AHPRA Pharmacist registration with no restrictions on practice
- Australian working rights, valid for at least 12-months from date of employment
- Demonstrated experience in clinical pharmacy services (medication review/reconciliation)
Desirable skills & qualifications:
- Experience in hospital pharmacy
- Exposure to palliative care or complex patient cohorts
