We are seeking an experienced Breast Care Nurse to join our innovative and dynamic team on a permanent part-time basis at Icon Cancer Centre, Midland. The Breast Care Nurse will coordinate patient care, aiming to enhance health outcomes throughout treatment and end-of-life care. Reporting to the Nurse Manager, this role is pivotal in ensuring seamless patient care delivery.
Key Responsibilities:
- Conduct comprehensive assessments and screening for clinical and supportive care needs, focusing on identifying at-risk patients.
- Ensure timely referrals to specialists, allied health providers, and support services as required.
- Foster continuity of care by establishing clear referral pathways and maintaining effective communication across healthcare settings.
- Provide consistent education and support to patients and their families.
- Implement evidence-based guidelines in the delivery of cancer care.
- Participate in service improvement initiatives to optimize care coordination and patient outcomes.
- Act as a liaison between patients, Medical Officers, and multidisciplinary healthcare teams.
Mandatory:
- Registered Nurse qualification with AHPRA registration.
- Minimum 3 years of clinical experience in breast cancer nursing.
- Demonstrated leadership and clinical expertise in oncology nursing.
- Ability to work autonomously and collaboratively within a team.
- Excellent interpersonal communication and problem-solving skills.
- Membership in relevant professional bodies (e.g., CNSA, COSA).
- Postgraduate qualification in oncology nursing or equivalent.
