The Opportunity 
We have an exciting opportunity for a Pharmacist to partner with our clinical team at Slade Pharmacy East Toowoomba, located in St. Vincent’s Private Hospital Toowoomba. About the Role
You will provide hands-on clinical pharmacy services, supporting patient care and working closely with a multidisciplinary team. This is the perfect environment to sharpen your skills and gain experience that will open doors to future opportunities.
  
Responsibilities
  • Conduct medication history interviews, medication reconciliation and pharmaceutical reviews at transfers of care
  • Dispensing and supplying medication and pharmacy products
  • Conduct therapeutic drug monitoring, monitor medication outcomes and adverse drug reaction management
  • Provide medication counselling & education to patients
  • Work cooperatively and effectively with multidisciplinary teams and provide advice for patient care.
  • Contribute to training and educational sessions for pharmacy, nursing, medical staff and allied health professionals
  • Support the quality use of medicines by participating in projects, audits, working groups or committees as required
  • Contribute to on-call and weekend services

Essential Requirements
  • A current AHPRA registration as a pharmacist.
  • Relevant experience in an Australian pharmacy setting.
